While our clinics experience the growing burden of depression within our communities, no one carries the weight of this more than the afflicted individual, and when they have felt both bad and brave enough to seek help and engage with treatment, only to find they don’t experience the expected benefits, this exponentially grows. In turn, the treating practitioner may understandably also feel anguish and overwhelm. What do you do when all your usual approaches have failed? This is the in-depth work-up of just such a case in a 41-year-old female whose depression & anxiety remained refractory in the face of 5 different psychiatric medicines and all the ‘usual’ alternative approaches. By employing a systematic framework, which can be applied to other mental health patients, we quickly come to see what is uniquely upstream of her downstream mood disturbance and, accordingly, change tack with treatment.
